Here’s how to enter your team!

 

1. Put your team together (4 people per team) and submit the registration form.  The deadline to register is May 20, 2017.  The registration fee is $25.00 per person (this will cover the cost of three games, shoe rental and one raffle ticket).

 

2. Once the registration form is sent, you may pay the fee via Paypal or mail your check payable to the Derek A Cooper Memorial Fund to:
